Who is Dr. Stork?

Dr. Jim Stork has joined our practice and now we want you to get to know him. Well, at least you can try! Just answer the questions in our office and the patient who answers the most questions correctly wins a $100 Movie Basket. One entry per patient, per visit. Contest ends September 30, 2011.
